The Quality Forum’s Patient-Centered Medical Home (PCMH) staff is conducting a series of webinars to educate health care professionals on medication adherence and caring for patients with hypertension and diabetes. The program is a partnership with DHH’s Office of Public Health geared toward improving clinical quality strategies in treating and tracking these conditions. The Louisiana Physician Orders for Scope of Treatment (LaPOST) Coalition is expressing its gratitude for the work being conducted by the Acadiana Health Coalition to promote the importance of care at the end of life. The Acadiana Health Coalition has ensured that patients’ medical preferences are being honored by standardizing usage of the LaPOST document and providing a script for care facilities to use when contacting Emergency Medical Services. 05-14-deadlineMedicare Hardship Exception Deadline July 1


Eligible professionals who were not able to participate in the 2014 Medicare EHR Incentive Program have until July 1 to apply for a hardship exception. Applications and instructions for individual and multiple Medicare eligible professionals are available on the EHR Incentive Programs website, and detail the specific circumstances that CMS considers as obstacles to achieving meaningful use. The changes go into effect January 1, 2016.
